Swelling after a root canal

A 34-year-old woman returns four days after completion of root canal therapy on the upper left first molar with facial swelling and difficulty opening her mouth.

History

· No relevant medical history, no medications
· Pain rated 7/10, worse when lying down
· Took ibuprofen 400 mg with partial relief

Examination & findings

· Diffuse swelling of the left cheek, no fluctuance
· Temperature 38.1 °C
· Maximum opening 28 mm
· Tooth tender to percussion, temporary restoration intact
